HOW TO TRANSCRIBE
MEDICATIONS
1. Number in list format if more than 1.
1.
Tylenol.
2.
Allopurinol.
3.
Aspirin
2. Type medication abbreviations as dictated;
p.o., b.i.d., t.i.d., etc.
3. Do not use dangerous abbreviations. The commonly used ones are:
h.s.
q.h.s.
q.d.
t.i.w
They should be expanded any where in the document to as
below:
h.s. - at bedtime
q.h.s. - nightly
q.d. - daily
q.weekly – every week
4. In Diagnosis/Diagnoses/Assessment part, all
these abbreviations should be expanded.
5. If dictated q 6 hours or q 8 hours, it should be transcribed as follows:
q.6 h.
q.8
h.
